Request Copies of Medical Records
If you need copies of your Medical Records, please attend the surgery and complete our Application for Access to Medical Records form.
Please be as specific as you can with the dates or information you require. You will need to bring photo ID (preferably passport or driving licence) when completing the form and again once your medical records are ready to be collected.
